EFFEXOR XR (venlafaxine hydrochloride) is a prescription drug that affects the levels of serotonin and norepinephrine, the two naturally occurring chemicals in the brain.

EFFEXOR XR is used for the treatment of the following DSM-IV category disorders:

• Major Depressive Disorder (MDD), which includes persistent occurrence of at least five of the following symptoms: depressed mood, markedly diminished interest or pleasure in usual activities, significant change in weight and/or appetite, insomnia or hypersomnia, psychomotor agitation or retardation, increased fatigue, feelings of guilt or worthlessness, slowed thinking or impaired concentration, a suicide attempt or suicidal ideation.
• Generalized Anxiety Disorder (GAD), characterized by excessive anxiety and worry, persistent for at least six months
• Social Anxiety Disorder (Social Phobia), marked or persistent fear of social or performance situations
• Panic disorder, characterized by recurrent and unexpected panic attacks or discrete periods of intense fear or discomfort.

Contraindications:

• Hepatic impairment or cirrhosis

• Bipolar disorder (manic depression)
• Family history of bipolar disorder or suicide
• Suicidal thoughts

The existence of these conditions may require your doctor to prohibit, make a dosage adjustment or call for special monitoring while taking EFFEXOR XR. This also goes the same with use for elderly, children and teenager.

Venlafaxine is in the FDA pregnancy category C, which means that it is not known whether it will be harmful to an unborn baby, or whether it can be passed on to nursing infants.

Interactions:

Do not take EFFEXOR XR (venlafaxine) without first talking to your doctor if you are taking the following medicines:

• Serotonergic drugs (including SSRIs, SNRIs and triptans)
• Drugs that impair metabolism of serotonin (including MAOIs)
• Monoamine oxidase inhibitors (MAOIs)
• Cimetidine
• Ketoconazole

Do not take EFFEXOR XR (venlafaxine) without first talking to your doctor if you are taking other prescription or over-the-counter medicines; including vitamins, minerals, and herbal supplements.

Avoid drinking alcohol.

Side effects:

Antidepressants increase suicidal thoughts or behavior, especially if children and adolescents with major depressive disorders and other psychiatric disorders. Talk with your doctor about this risks associated with antidepressant treatments. Close patient monitoring is highly recommended, and family members or attendants should closely monitor changes in mood and occurrence of other symptoms.

Treatment with EFFEXOR XR may lead to the development of the potentially life-threatening Serotonin Syndrome, particularly with simultaneous use of serotonergic drugs (including SSRIs, SNRIs and triptans) and with drugs that impair metabolism of serotonin (including MAOIs).

If you experience the following conditions, inform your doctor immediately:

• Mental status changes (agitation, hallucinations, coma)
• Autonomic instability (tachycardia, labile blood pressure, hyperthermia)
• Neuromuscular aberrations (hyperreflexia, incoordination), and/or
• Gastrointestinal symptoms (nausea, vomiting, diarrhea).

Other less serious side effects include:

• Abnormalities of sexual function (abnormal ejaculation, impotence in men, orgasmic dysfunction)
• Gastrointestinal complaints (nausea, dry mouth, anorexia, constipation, flatulence)
• Dizziness, somnolence, abnormal dreams, tremor, nervousness
• Abnormal vision
• Insomnia
• Loss of appetite
• Sleepiness
• Sweating
• Weakness
• Yawning

Side effects other than those listed here may also occur. Talk to your doctor about any side effect that seems unusual or bothersome.

Dosage:

Take EFFEXOR XR exactly as prescribed by your doctor. Frequency and dosage varies with type of condition. Take EFFEXOR XR with food, either in the morning or in the evening at about the same time each day.

Swallow the capsule whole. EFFEXOR XR should not be divided, crushed, chewed, or placed in water.

For patients with difficulty swallowing, EFFEXOR XR capsule can be opened and the granules inside can be mixed with one tablespoon of applesauce. The applesauce should not be hot and should be soft enough to be swallowed without chewing. Granules should not be chewed or crushed. This mixture should not be stored for later use.

Storage:

Store EFFEXOR XR at controlled room temperature, between 20° to 25°C (68° to 77°F).
